Abstract

Grade 2 hypertension is a major health problem associated with a high risk of cardiovascular complications. The implementation of standardized nursing care is crucial for controlling symptoms and preventing complications. Although national nursing care standards in Indonesia—namely the Indonesian Nursing Diagnosis Standards (SDKI), Indonesian Nursing Intervention Standards (SIKI), and Indonesian Nursing Outcomes Standards (SLKI) have been established, studies describing their comprehensive implementation in regional hospitals remain limited. This study aimed to describe the implementation of SIKI–SLKI-based nursing care for a patient with grade 2 hypertension in the Mawar Ward of Mamuju Regional General Hospital. A descriptive case study design was employed involving one patient diagnosed with grade 2 hypertension. Data were collected through anamnesis, head-to-toe physical examination, three days of clinical observation, and review of nursing documentation. The nursing process included assessment, formulation of nursing diagnoses, planning of interventions, implementation, and evaluation in accordance with national nursing standards. Four nursing diagnoses were identified: acute pain, disturbed sleep pattern, activity intolerance, and knowledge deficit. The implementation of SIKI-based nursing interventions led to clinical improvements, as evidenced by reduced pain intensity, improved sleep quality, increased activity tolerance, and enhanced patient and family understanding of hypertension and its management. The implementation of SIKI–SLKI-based nursing care was effective in addressing the main nursing problems of a patient with grade 2 hypertension during the hospitalization period. These findings highlight the importance of consistent application of national nursing care standards in regional hospitals to improve the quality of nursing services.
