Abstract

Psychological adaptation disorders during pregnancy potentially threaten maternal and fetal health and correlate with stunting risks due to prenatal emotional instability. Current antenatal care is still dominated by physical approaches, often neglecting the psychosocial dimension since early pregnancy. This study aims to analyze the effectiveness of the husband and family involvement intervention model (Family-Care 4P: Presence, Psychoeducation, Participation, Positive Reinforcement) in improving the psychological adaptation of pregnant women. This quantitative study applied a Quasi-Experimental design with a Non-equivalent Control Group Pretest-Posttest Design involving 30 subjects selected through the consecutive sampling technique. The study was conducted at two locations: Batua Community Health Center (Puskesmas) as the intervention group (receiving Family-Care 4P model education using booklet media) and Tamalate Community Health Center as the control group (receiving standard ANC services). The research was carried out between April and May 2026 utilizing the Family-Care 4P questionnaire, the Edinburgh Postnatal Depression Scale (EPDS), and the Family-Care 4P booklet as research instruments. Statistical data analysis was performed using non-parametric tests, specifically the Wilcoxon Signed-Rank Test and Mann-Whitney U. The results showed a significant decrease in the mean EPDS score within the intervention group from 7.133 to 4.800 (p = 0.001), whereas the control group experienced a slight increase in scores from 7.200 to 7.667 (p = 0.236). The inter-group analysis demonstrated that the husband and family involvement intervention was significantly effective in improving the psychological adaptation of pregnant women (p = 0.001). The Family-Care 4P model involving husbands and families is proven effective in improving the psychological adaptation of first and second-trimester pregnant women, thereby it is recommended for integration into routine antenatal care starting from early pregnancy at community health centers.
